Which OASIS version do we use for discharge?

Q: This question is in regard to the OASIS-C and discharge. When we discharge our current patients with a start of care date in November and December 2009,  do we use the OASIS-B1 or OASIS- C? Synergy is telling us to use OASIS-C, but how can we answer questions we never answered in the original OASIS?  This last time we changed OASIS they had us using the old discharge forms until all cases were discharged.

A: The date the assessment is completed (M0090) dictates which assessment is used. All assessments completed on or after January 1, 2010 require documentation of the OASIS-C assessment. Clinicians should answer items on the assessment that reflect the patient’s assessment findings on the day of assessment based on established conventions. In evaluating items such as M2400, Intervention Synopsis, if the interventions were not included on the plan of care and implemented, a response of “No” would be appropriate.
