WOCN Tool for Pressure Ulcers on OASIS-C

Many home health professionals have questions about the changes to the data elements on pressure ulcers. Ask OASIS-Central gets a lot of questions about this subject.  The new data elements are looking for more detail about the pressure ulcers and this forces caregivers to have a true understanding of pressure ulcers and the terms that are associated with them.

If you haven’t seen it already, check out this great tool from the Wound, Ostomy and Continence Nurses Society.  This handy document gives in-depth definitions of pressure ulcer terms that appear on OASIS-C.  For the visual person, it also features drawings.

    It is also helpful to look at the pressure ulcer staging info at http://www.npuap.org These are the definitions that were used by CMS when writing the pressure ulcer items. They have good picture, too :-)
