Public Reporting Facts, Part 2

Last week’s post offered insight into different publicly reported outcomes. Here is a detailed list of exactly what will be reported.

What will be reported?

A. Process (7 domains, 13 measures)

B. Potentially avoidable events (clinical domain, one measure)

C. Utilization outcomes (utilization domain, two measures)

D. End result outcomes (clinical and functional domain, seven measures)

A. Process Measures (13)

Timely Care:

Timely initiation of care

Assessment:

Depression assessment conducted

Pain assessment conducted

Falls risk assessment for >65

Pressure ulcer risk assessment conducted

Education:

Drug education on all meds provided

Care planning:

Pressure ulcer prevention included in POC

Care plan implementation:

Diabetic foot care and education implemented

Heart failure symptoms addressed

Pain interventions implemented

Prevention:

Flu immunization

Pneumococcal vaccination

Pressure ulcer prevention implemented

B. Potentially avoidable events (1)

Increase in number of unhealed pressure ulcers (pressure ulcer develops while under home health care)

Tip: Development and implementation of comprehensive pressure ulcer prevention program is key

C. Utilization Outcomes (2)

Acute care hospitalization (M2410)

ER department care without hospitalization:

Beginning in 2011. HHC will report these two utilization outcomes

Discharged to community had been utilization outcome generated from OASIS-B1 data, but CMS eliminated it

D. End Result Outcomes (7)

Improvement in ambulation/locomotion (M1860)

Improvement in bathing (M1830)

Improvement in bed transfer (M1850)

Improvement in management of oral meds (M2020)

Improvement in dyspnea (M1400)

Improvement in pain interfering with activity (M1242)

Improvement in status of surgical wounds (M1342)
