Could you please specify the procedures that should be taken when an OASIS discharge assessment can not be done and the last SN visit must become the discharge?

Q: Please specify the procedures that should be taken when an OASIS discharge assessment can not be done and the last SN visit must become the discharge. Should the schedule be revised? Is this discharge billable or non-billable?

A: If the patient is discharged unexpectedly, the discharge assessment would either need to be documented by the qualified clinician who made the last visit to the patient’s home, or a non-billable visit would need to be made to the patient’s home within 48 hours of the agency learning of the need to discharge. A discharge visit would only be billable if an ordered, skilled service was provided and the patient qualified for homecare services.
