A: If the patient returns to the agency during the last five days of the episode after a qualifying inpatient facility admission, a resumption of care assessment would be completed, along with enough details to generate a new plan of care and orders for the new certification period. A recertification assessment would never follow a transfer assessment.

    I would like to know what to do if the patient returns to the agency after an hospitalization on day 61 or 62?

    If the patient returns home on day 61 or 62 after an inpatient facility stay in which a recertification assessment was not completed during the last five days of the episode, a start of care assessment is required. A discharge assessment should not be completed. The clinician simply needs to complete agency-specific discharge paperwork.
